The Medical Center Home Care Program provides tailored medical care under the direct supervision of the referring physician, all in the comfort of your home. Our goal is to promote recovery, independence, and improved quality of life while ensuring safety and comfort. Our service area includes Allen, Butler, Edmonson, Simpson, and Warren counties.
Skilled Nursing
- Comprehensive care including IV therapy and wound care
- Cardiac care for hypertension, congestive heart failure, chronic obstructive pulmonary disease, and other respiratory diseases
- Patient and family education
- Pain and medication management
- Assessment of surgical sites
- Disease management programs
Physical Therapy
- Individualized exercise programs
- Fall prevention and home safety education
- Improvement of strength, mobility, and functional abilities
- Recovery following surgery
- Increased independence and resumption of normal activities
Occupational Therapy
- Self-care training to improve independence and safety (dressing, bathing, feeding)
- Improvement of higher-level home tasks (cooking, laundry, housekeeping)
- Medication management
- Education on energy conservation, coping with a disability, and joint protection techniques
- Recommendation and education on adaptive equipment
Speech Therapy
- Improvement of communication, language, and executive functioning skills
- Improvement of personal safety awareness
- Diet modification and recommendations
- Safe swallowing education to prevent aspiration
- Patient and family education for dementia care and disease processes
- Augmentative and Alternative Communication (AAC) and communication device evaluation/treatment
- Dysphagia evaluation and treatment
