The Dental Clinic

The Dental Clinic provides basic services at reasonable cost to people without private dental insurance and whose family income falls below 250% of the national poverty guideline. Patients must live in Allen, Barren, Butler, Edmonson, Hart, Logan, Metcalfe, Monroe, Simpson or Warren counties. Patients are asked to provide a copy of their current Federal Tax 1040 form on first visit, or a Federal Form 4506-T (which we will provide) to help us qualify you for eligibility. Patients are seen by appointment only and a minimum service fee is billed at the time of appointment and based upon your ability to pay.

Help available for orthodontic Care

The Malchow Orthodontics Endowment Fund through Med Center Health Foundation makes available quality orthodontic care for children of families with financial need residing in Warren and surrounding counties. Services are provided by licensed orthodontists who maintain their principal office and place of practice in Bowling Green, Kentucky.
