THANK YOU for your Employees Contributing to Help Others (ECHO) membership! Because all administrative costs of the Foundation are funded by Med Center Health, 100% of your gift supports Med Center Health Foundation initiatives such as Community Clinic/The Dental Clinic, Guardian Angel Program, and Hospitality House. Your donation is tax-deductible and will be listed at year end on your W-2.

Please fill out the pledge form below, selecting the amount you wish to give and where you would like your donation to go. You may also download the form, fill it out and send it via interoffice mail it to Kathy Smith, Director of Annual Giving with Med Center Health Foundation.
