Your Quick Look into Safety at Med Center Health Bowling Green

At Med Center Health, safety and high reliability are more than just goals. They are our promise to every person who comes through our doors. This Safety Snapshot keeps you informed and involved in our ongoing initiatives.

INcident Reporting

  • Accessed through MyMCH Portal
  • All reports are read and sent to the appropriate leaders and departments for follow up.
  • Reports are run to identify trending issues where we may need to focus on policy changes, education, or other interventions.
  • When near misses or patient harm occurs, our Patient Safety Officer and a team get together with those involved to identify areas where our system has failed. We use these meetings to identify practice changes to avoid the issue happening again.
  • These meetings are non-punitive. This means we do not “point fingers” or place the blame on any individuals. We look at where and how we can improve processes.
  • You can contribute by keeping your manager up to date on safety concerns and filing incident reports for all accidents, errors, good catches, or any unusual, unsafe, or unanticipated events.
