Med Center Health Glasgow offering free sports physicals for middle and high school students

Med Center Health Glasgow will be offering free sports physicals on Monday, June 30, from 4:00 p.m. – 6:00 p.m. at Med Center Health Glasgow. The address to the facility is: 401 S L Rogers Wells Blvd., Glasgow, KY, 42141.

The sports physicals will be free to all Glasgow/Barren County middle school and high school students. Prior to the exam, KHSAA forms PPE01 and GE04 must be completed by both the athlete and a parent or guardian, if the athlete is under 18. A link to those forms can be found here:

For a faster and easier experience, please bring the completed forms with you. Copies will also be available on-site but will need to be signed by a parent or guardian, if the student is under 18.
