Fitness Facility Reimbursement Program

As the newness of the new year starts to wear off and we head into March, which ends the first quarter of 2023, Employee Health wants to remind everyone to take advantage of the Fitness Facility Reimbursement Program. The Fitness Facility Reimbursement Program is part of the MCH Get Fit Club. This program offers MCH employees an opportunity to be reimbursed $50 each quarter. Employees will need to be a member of a fitness facility of their choice, complete the Fitness Facility Form and provide documentation of participation. Twenty-four visits are required per quarter to be eligible for the reimbursement. The completed forms will need to be submitted to Employee Wellness at EHNH@mchealth.net.

If you are new to the Fitness Facility Reimbursement program, it is easy to start. Just follow these four easy steps!

  1. Enroll in a Fitness Facility of your choice.
  2. Download the Fitness Facility Attendance card, visit the Get Fit Club Website (icon on your desktop), or call (270) 745-1263 and have a wallet card mailed to you.
  3. A Fitness Facility staff member should stamp, initial or endorse the attendance card each time you attend, or you may attach your attendance roster.
  4. After 24 documented visits, submit the Attendance Card/Form through inter-office mail to MCH Employee Wellness, scan and email to EHNH@mchealth.net or fax to Employee Wellness at 270-796-2528. When we process the form, you will receive a $50 reimbursement check toward your membership fees (forms are processed quarterly). No more than one reimbursement check will be issued per employee per calendar quarter.
