Med Center Health Partners – for Physicians

Med Center Health Partners is a Clinically Integrated Network of healthcare providers, working together under a physician-led structure to improve quality and reduce the cost of healthcare by building a partnership of providers that is patient-focused, value-based and physician-led.


What Does it Mean to be a Participating Physician? Med Center Health Partners providers accept accountability and will lead by example. They agree to:

  1. Active participation in the clinical integration program which requires:
    • Submitting quality/clinical data to the network
    • Completing technology training
    • Reviewing data on a monthly basis
    • Engaging on quality and cost-efficiency opportunities/initiatives
  2. Be measured against Med Center Health Partners vetted quality measures.
  3. Comply with Med Center Health Partner’s Policies and Procedures.

Quality Improvement and Accountability

Med Center Health Partners has established its Quality Program through a collaborative effort led by its Medical Director and members of the Quality Committee. This program identifies specific evidence-based measures covering all specialties in both the inpatient and ambulatory settings. Med Center Health Partners providers have agreed to submit practice data for this ongoing improvement effort and to participate in efforts to improve the best outcomes for our patients.

Hospital Quality & Efficiency Program

The Hospital Quality and Efficiency Program (HQEP) is a partnership between The Medical Center and Med Center Health Partners to achieve quality improvements and care efficiencies within the hospital setting and the Med Center Health medical plan. Together, our physicians and the hospital are working to support:

  • Quality and safety across the continuum of care
  • An enhanced patient experience
  • Increased efficiency

Data Analytics Platform

Med Center Health Partners uses Crimson as its data analytics platform. Crimson offers a comprehensive set of IT Solutions designed to:

  • Provide physicians an unprecedented “line of sight” across the continuum
  • Provide “actionable dashboards” facilitating changes in practice patterns
  • Provide population health tools enabling Enspire to identify:
    • High risk members
    • Rising risk members
    • Gaps in care & creates a way to effectively engage members/caregivers
  • Provide a workflow tool to engage patients and their healthcare team to improve quality and outcomes

Over 900 hospitals and thousands of physicians currently use Crimson and more than 26% of U.S. hospital admissions are in Crimson.

Bundled Payment Programs

The Medical Center is a Model 2 Participant in the Center for Medicare Services’ Bundled Payment for Care Improvement Initiative. Our physicians are working with the Hospital to deliver better patient outcomes and efficiencies for patients with these conditions from the acute care stay through 90 days post discharge:

  • Acute myocardial infarction (heart attack)
  • Percutaneous coronary intervention (PCI)
  • Simple pneumonia and other respiratory infections

Population Health

Population health management focuses on preventative care to keep healthy patients well – but also on high-risk patients with chronic conditions. Providers use data collected from all care settings to understand gaps in care and the challenges high-risk patients face in the management of their acute or chronic conditions. When these efforts are successful, the need for expensive interventions like emergency department visits, hospitalizations, imaging tests and procedures is lessened.

Med Center Health Vaccine Clinic Update
The COVID-19 vaccine and booster are available at Medical Center Urgentcare located at 291 New Towne Drive in Bowling Green for those 12 and older. For those under 12, the vaccine is available at Med Center Health Primary Care Bowling Green.
