Care Management – Coach and Cheerleader in one!
There are times when we all can benefit from a coach and cheerleader to help on the journey to good health. Well, Med Center Health’s Care Manager, KenYatta Williams, RN, BSN, is both! If you have MCH health insurance, KenYatta will help guide you, encourage you and celebrate your successes with you. The best part? It doesn’t cost you a dime.
“Our goal is to help employees live a healthy and balanced life,” KenYatta says. “We are all very busy and it’s easy to slip into a daily routine that’s not as healthy as it could or should be. Many of our employees struggle with high blood pressure, diabetes or some other health issue.”
KenYatta is here to help you and to provide support and encouragement. She helps with lifestyle advice, including diet, exercise and stress relief. Her approach is holistic – touching on mental health, physical health, even spiritual health – and all of it is employee-focused and based on what the employee wants to achieve.
“I know I can help people because I am one of you,” she says. “I understand. I’m right there with you. I can help you learn about things you can incorporate into your lifestyle and help you manage your medications.”
Working alongside KenYatta is Medical Assistant Mary Rose. She is usually the first one an employee will talk to in order to set up a consultation with KenYatta. “We both believe your health is your wealth,” Mary says. “We want everyone to be wealthy! My job is outreach, to get people in to see their primary care provider for an annual wellness visit and to get on track for screenings.”
If interested, employees can reach out directly to Mary and KenYatta. “We can meet them at their physician appointment,” KenYatta says, “or I can travel to see them on their lunch break. I try to make it flexible because everyone works.”
Care Management is available to all employees and dependents who are covered by the MCH medical plan. KenYatta and Mary are coordinating with primary care providers to follow-up and carry through on the primary care provider’s care plan. KenYatta says, “We’re on the same team with your primary care provider, and we want to help you on your journey to good health.”
